Online application for Income Tax and Tax Payment certificates (for municipal and national health insurance taxes)

所得課税証明書・納税証明書（市県民税・国民健康保険税）のオンライン申請

You can apply online for an Income Tax Certificate and Tax Payment Certificate.

Once your application has been received and verified, you will receive a payment request email from the Tax Division. 

Please make your payment by Credit Card from the URL in the payment request email.

The certificate will be sent by post after the credit card payment has been completed.
Please note that the certificate cannot be sent until payment is complete, so be careful if you're in a hurry.

※Online submissions by proxy or on behalf of a corporation aren't permitted. 

Please apply by mail or visit the counter at Kuwana City Hall.

Application process

Please prepare the following documents and other documents.
The address to which the documents should be sent is the address stated on the identity document.

1.Smartphone or computer
2. Image of the identity document
3. Credit card

Caution

(Note 1) The application will be canceled if payment isn't confirmed within 7 days.

(Note 2) Canceling Applications after payment cannot be accepted.

(Note 3) Credit card payment receipts will not be issued.

(Note 4) If you have set your email address to reject emails, please ensure that you can receive the following email address.

Procedure after entering application details

Confirm the estimated amount.

Press [→ Proceed to confirmation screen]. ( Image on the left)

Check the information entered and press [→Submit] to complete the application. (Image on the right)

When you receive an email confirming the payment details

Enter your password. (★) Press [→Proceed with this application].

Check that the application is correct and press [→ Proceed to payment].

Enter your Credit Card details.

1. Card number
2. Cardholder
3. Expiry date
   Month/Year
4. Number of Payments (One-time Payment)
5. Security Code Number

Press [→ Proceed to confirmation screen] to continue.

If everything is OK, press [Pay].
